Summary

  • JOB SUMMARY
    • The Care Coordinator (CC) is primarily responsible for coordinating the care of patients designated as Health Care Home (HCH) patients by acting as a support to both the patient/family and the HCH provider. The Care Coordinator works directly with patients and their family members to facilitate the prescribed care plan utilizing available resources and services to meet the needs of the patient. Provides support in other clinic areas as needed. The Care Coordinator functions under the direction of a physician, mid-level or RN. The Care Coordinator supports Population Health Initiatives. Supports Annual Wellness Visits, Advanced Care Planning and Transition Care Management processes as assigned. Work is aligned with division’s strategic goals and objectives.
  • MINIMUM QUALIFICATIONS
    • Education: Successful completion of an accredited training program preparing applicant for a position with responsibilities in care coordination. Educational preparation may include programs in medical assisting, nursing, education, or human services.
      Experience: Three (3) years healthcare or human service experience.
      Licensure/Certification/Registration: N/A
  • PREFERRED QUALIFICATIONS
    • Education: Approved vocational/technical school for healthcare or human service related field.  Prefer LPN.
      Experience: Previous experience working in Public Health or with Community Resources; previous experience with case management or coordination.
      Licensure/Certification/Registration: Current license/certification in subject area.

    • Perform tasks within scope of education and Scope of Nursing Practice; maintain confidentiality; communicate effectively, both oral and written; support St. Luke’s goals; learn and perform basic computer keyboard operations.

    • READING - Intermediate: Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.
    • WRITING - Intermediate: Ability to write routine reports, correspondence, or procedures.
    • SPEAKING - Intermediate: Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
    • MATHEMATICAL SKILLS - Intermediate Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals and to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to compute rate, ratio, and percent and to draw or interpret bar graphs to apply concepts of basic algebra and geometry.
    • REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
